NationStates suffered a server breach on Jan 27 (or 28, depending on where you live), whereby an unauthorized person gained server entry and copied data offsite. The site was down for approximately 11 days, during which we audited our software, upgraded password security, and built an account recovery mechanism.
Because your password was potentially compromised, in order to protect your account, we must force a password reset on all nations.
To login:
If you supplied an email address, you’ll be prompted to generate an email password reset link.
Otherwise, if you are logging in from the same location we’ve seen before, you will be prompted to change your password.
Failing that–if your nation has no valid email and you’re logging in from an unseen location–you will be invited to submit a manual review request. If this succeeds, moderators will email you a password reset link.

If you have multiple nations:
For a lot of nations, once you’ve reset one of them, use Mass Password Reset for the others
Alternatively:
First try to login to the nation you last used before the site went down, so we can try to reverify your previous session.
If possible, before starting recovery, be in a location that you’ve used to access NationStates before.
If you have a World Assembly member nation, try to recover it first.
After a successful recovery, remain logged in while attempting to recover more nations via the login page.
